Sports Physicals

Syringa Hospital & Clinics will be offering physical examinations by appointment only for all students for the purpose of participating in sports activities throughout the 2020-2021 school year.

Physicals will be offered at both Grangeville and Kooskia Clinics

The cost is FREE with a suggested voluntary donation of $25.
100% of all donations received will go directly back to that child's school.


We highly encourage you to complete the forms prior to coming to your appointment. Forms are available by clicking on the links below.

What to Bring:

The Idaho High School Activities Association - Idaho Health Examination and Consent Form with page 1 completed.
​Click here for Kooskia or here for Grangeville. Forms will also be available at the clinics. The consent portion MUST be signed.

Each participant must also have a parent or guardian complete the Physical Release Form, click here. This allows us to release the physical exam information to your child's school if the original form given to the student is lost.

Please wear loose, comfortable clothing.
