Patient Care Coordinator

Syringa Hospital & Clinics is accepting applications for a Full-Time Patient Care Coordinator. This position provides coordinated care to patients with chronic care conditions and or behavioral health needs by developing, monitoring, and evaluating interdisciplinary care. The Patient Care Coordinator coordinates team-based care to provide health services to individuals, through effective partnerships with patients, their caregivers/families, community resources, and their physician. Facilitates a “shared goal model” within and across settings to achieve coordinated high-quality care that is patient- and family-centered. RN, LPN, or Social Worker required. Previous Care Management or Case Management experience preferred. Must be competent in interpersonal relations. Must possess the ability to be flexible and prioritize work tasks, etc. Must have ability to work independently with minimal supervision. Previous experience with mobilizing community resources, navigating patients through the healthcare continuum, and working with disparate populations preferred.

Unit Clerk/CNA

Syringa Hospital & Clinics is accepting applications for a Part-Time Unit Clerk/C.N.A. This position Responsible for a variety of support duties at the Nursing desk which includes notifying laboratory and radiology of tests, answering telephones, addressing call lights, monitoring the EMT radio, monitoring the cardiac monitor, maintaining various logs, keeping the EMS resource site up to date, aid in patient discharge or transfers by copying selected parts of the chart, making sure all patients have an identification bracelet, scheduling outpatient procedures when appropriate, scheduling follow up appointments for discharging patients, working closely with floor staff, maintaining order and neatness at the nurses station and providing miscellaneous support duties to the Charge Nurse to provide organized, quality patient care. Assisting on the floor as a C.N.A when needed, or ER. High school graduate preferred. Completion of Idaho approved nursing assistant training program or actively enrolled in an Idaho approved nursing assistant training program. Clinic Supervisor-RN/LPN

Syringa Hospital & Clinics is accepting applications for a Full-Time Clinic Supervisor-RN/LPN. This position ensures that quality clinical assisting / nursing care is rendered to all patients in accordance with clinic policy and procedures. Supervises Clinical Assistants, providing direction to staff and ensuring that tasks are done efficiently and correctly for the highest benefit of the patient. RN or LPN required. Dietary Aide

Environmental Services Aide

Discharge Planning RN

Syringa Hospital & Clinics is accepting applications for a Full-Time Discharge Planning Registered Nurse. The Discharge Planner is responsible for facilitating continuity of health care for each admitted patient through the discharge planning process. Current RN licensure, or eligible, with the Idaho State Board of Nursing; or Licensure (BSW) in social work in State of Idaho or Bachelor’s or Associates Degree in related field with experience in healthcare field. At least three years clinical experience or discharge planning experience preferred.

Patient Financial Services Clerk

Syringa Hospital & Clinics is accepting applications for a Full-Time Patient Financial Services Clerk. This position is responsible for posting all insurance payments received to patient accounts, client payment posting, and self-pay payment posting. Validate, and correct as needed, auto-posted electronic payments. Separates and monitors files for all insurance companies so readily available for fiscal year end needs. High School diploma or equivalent preferred. 1-2 years of health care industry experience preferred. Must be able to handle tasks quickly and professionally.

Coding Specialist

Syringa Hospital & Clinics is accepting applications for a Full-Time Coding Specialist. The primary responsibility of the Coding Specialist is coding and charge capture for hospital services. High School graduate preferred or equivalent with computer skills required. Coding certification required and 3-5 years health care industry experience in coding. Knowledge of billing preferred. Completion of medical terminology, anatomy and physiology courses required.

Clinic Medical Assistant

Syringa Hospital & Clinics is accepting applications for a Full-Time Clinic Medical Assistant. The medical assistant will, under the direct supervision of a licensed practitioner, assists the health care provider in delivery of patient care in a clinic setting. Coordinates nursing care for patients according to established procedures. Certified Medical Assistant preferred. Experience in clinical practice and familiarity with medical terminology preferred.

Physical Therapist

Syringa Hospital & Clinics is accepting applications for a Full-Time Physical Therapist. Physical therapist provides physical therapy services including patient evaluation, development and administration of an intervention plan, documentation and communication with physicians and other referral sources, and clinical guidance to non-professional personnel. Provides therapy treatment, in inpatient and outpatient setting as well other practice environments as necessary. Bachelor's degree from an accredited school of physical therapy and state licensure to practice physical therapy by the Idaho State Board of Medicine required. Experience/expertise in multiple treatment settings preferred.

Utilization Review RN

Syringa Hospital & Clinics is accepting applications for a Part Time Utilization Review RN. The UR nurse is responsible for analyzing hospital inpatient records and applies admission, treatment, and length of stay criteria in conformance with government and insurance company reimbursement policies. Monitors private and government insurance standards pertaining to admission, treatment, and length of stay of patients. Compares inpatient medical records to established criteria, conferring with medical, nursing and ancillary staff to evaluate treatment and length of stay. This position requires a degree in nursing and clinical care experience. Hospice- RN

Occupational Therapist

Syringa Hospital & Clinics is accepting applications for a Full-Time Occupational Therapist. The staff Occupational Therapist provides occupational therapy services including patient evaluation, development and administration of an intervention plan, documentation and communication with physicians and other referral sources. Minimum of Bachelors degree from an accredited school of Occupational Therapy required. Completion of National Certification Examination for OTR and state registration to practice occupational therapy by Idaho Bureau of Occupational Licenses required.
